"When it was time for my mother to be discharged from the hospital I had two choices, a sub-acute facility or home, if I selected the sub-acute I would not be able to see my mom or help her with the restrictions because of COVID, so I elected to take her home. Placement Manager Jason was detail oriented and responsive! He understood grandma was being placed in hospice care and heard the worry in my voice when I explained we really needed an overnight person to start same day!
